Overview

What is Gout and High Uric Acid?

Gout is a crystal arthropathy caused by the deposition of monosodium urate crystals in joints, tendons, and surrounding tissues when serum uric acid levels remain persistently elevated. It presents as sudden, excruciating attacks of joint pain — most classically in the big toe — but can involve ankles, knees, and wrists.

Scope

Conditions Treated

We offer targeted therapies for a wide spectrum of related autoimmune and inflammatory conditions to ensure comprehensive relief.

  • 01Acute Gout Attacks and Flares
  • 02Chronic Tophaceous Gout with tophi deposits
  • 03Intercritical Gout (between attacks)
  • 04Gout associated with CKD or medications
  • 05Asymptomatic Hyperuricemia with high cardiovascular risk
Eligibility

Who May Need This?

1

Patients with sudden, severe big toe or ankle pain, especially at night

2

Individuals with serum uric acid above 7.0 mg/dL

3

Those with chalky white lumps (tophi) under the skin

4

Patients with recurrent kidney stones due to uric acid

5

Individuals on diuretics, aspirin, or transplant medications causing secondary gout

Mechanism

How Does It Work?

Acute flares are terminated rapidly using Colchicine and anti-inflammatory protocols. Once the flare subsides, we initiate Urate-Lowering Therapy (ULT) with Febuxostat or Allopurinol, targeting serum uric acid below 5.0–6.0 mg/dL. At this level, existing urate crystal deposits in joints dissolve over 6–24 months, permanently eliminating future attacks.

Week 2–4: Uric acid testing and ULT initiation once flare resolves

  3. 3

Month 3: Recheck serum uric acid to confirm target achievement

  4. 4

Month 6–12: Gradual crystal dissolution and prevention of new deposits

  5. 5

Long-term: Yearly monitoring to maintain uric acid in target range
